Dear Lord,
I'm sorry for the way I have lived my life.
For my actions...My words, both written and spoken, my thoughts and attitudes, my broken promises and the way I have let people down.  

Please forgive me.  

Please help me to repent- to turn away from all the wrong things in my life. Please now come into my life now by your Spirit to be my Lord and Saviour forever and help me to be the person you want me to be.  Amen
