Meaningful Moment’s Podcast: Ep. 9 with Care Coordination President Julie McCarter

In 2023 Help at Home launched our innovative Care Coordination program that builds upon our unique caregiver/client longitudinal relationships. In the past year our dedicated Care Coordination team has adapted to myriad learnings, and celebrated even more successes. 

“The impact we created is real,” said Care Coordination President Julie McCarter. “We have proven it is incredibly important to infuse everything we do through a health equity lens. We are closing gaps in care, we are increasing utilization in primary care services and decreasing emergency department visits. The impact is happening across our populations.”

2023 was a landmark year for Care Coordination and 2024 is shaping up to be even more impactful.
