Safety Moment - Investigations Learn and Corrective Actions Fix

This is the most important safety moment in a while.  

As goofy as this sounds - we tend to think of an investigation as a "fix."  Investigations are never fixes...investigations are done for one reason:  To learn and improve (that seems like two reasons - just go with it).   

Corrective actions fix.     Corrective actions are the result of learning.  Stop saying your company investigates in order to stop re-occurance... that is wrong.
