Swing Bed Services at OSF Saint Luke Medical Center are now called 'Closer to Home.' This is the service that prepares long term patients to be able to safely and carefully return home. Swing Bed Services, now Closer To Home, are intended to provide needed preparation and guidance for those who spend time in the hospital on how to care for themselves at home alongside family, their at home support system. Lisa Garcia from OSF Saint Luke Medical Center joined Samantha Rux on Wake Up Tri-Counties to talk about helping long term patients safely and effectively transferring home.
