How do you create a culture of Accountability?

My Vistage Group recently had a presentation from former USAF Pilot Robert Teschner - Call sign CUJO. He talked to us about the importance of Debriefing as a path to Accountability. He described to us his experience as a Fighter Pilot where they debrief after every mission. Their lives depend on being Open and Honest about what when well and what didn't. Too often in business we only debrief when things don't go well and we want to hold someone accountable.
