How do you ask for feedback after an interview, and when is it appropriate to ask?

I'm a big fan of asking for feedback, even on things like my podcast series and live streaming. Feedback is always helpful. Some companies are reluctant to provide good (if any) feedback, and some companies LOVE to give feedback. Why aren't companies more consistent about this?
