Today we’re talking about 2 hour response teams who work in the community supporting admission avoidance where possible. Typically they work with patients for short periods of time 24-72 hours where a person is at risk of an acute admission or readmission to hospital.
Teams have a multitude of names, rapid response, crisis team, urgent community response and more so to keep things generic and applicable to you where ever you are we’ve used the phrase 2 hour response team. Do go and find out what services near you are called and how they work!
