There are laws that protect your bodily autonomy--that prevent doctors, nurses, hospitals, administrators, etc. from administering treatments you decline. But if you do not know HOW to protect your choices, if you do not have an advocate to defend those choices, and if you do not have the legal documentation delivered to the right people, your chosen care may not be administered and your wishes not respected.
