A Healthier Rural America - Scale the Action: Interview with Dr. Phil Polakoff & Matthew Probst

Dr. Phil Polakoff and Matthew Probst are paving the way for drastically improved rural health outcomes. 

Dr. Phil Polakoff is the innovative creator behind the National Institute for Rural Health, and he’s a passionate disruptor with a purpose. Matthew Probst is the Medical Director and Chief Quality Officer at El Centro Family Health, and he’s had tremendous success improving the health of his community in Northern New Mexico through the 100% Community initiative. 

Together they work as allies toward the improvement of rural health, moving away from “sick care” and toward prevention and wellness. Through community-based programs and involvement, as well as leveraging data, spirit, hope and energy, true transformation is on the horizon.

Through the National Institute for Rural Health, Phil and the team are working to streamline governmental processes and funding, helping areas nationwide, like the area Matthew serves in New Mexico. 

Matthew’s community’s implementation of the 100% Community initiative  serves as a great case study of success using grassroots efforts to help the community improve access and outcomes.
