Are you intentionally actively pursuing wellness? When was the last time you evaluated your wellness? So many people are struggling to get on track with taking care of their minds, their spirits, their bodies, emotions, etc. Will you take time to engage in this reflection and choose to actively pursue wellness!!? 1. How often have you felt happy?

2. How have you been able to problems that you have maybe addressed in therapy or problems in general?

3. How often have you felt good about yourself?

4. How often have you felt relaxed?

5. How often have you been sleeping well?

6. How often have you felt satisfied with your relationships?

7. How often have you had healthy habits (diet, exercise, hygiene,etc)?

8. How often have you been able to focus?

9. How often have you made good decisions ?

10. How often have you been able to fulfill your obligations (work, school, etc)
