Meetings should have agenda's
Meetings should have a time scope (start and stop time)
Meetings should be as productive as possible.
Meeting are already in-efficient. If we were perfectly in sync and on the same page, there would never be a need for meeting.
So, we take people from their productive format (whatever their are paid by the company to do) and we bring them to a meeting about inefficiency. So we make them more inefficient to make them more efficient.

So speakers drone on
Management and employees in one place
So people want to "show/display their worth"
Too much feedback to simple questions
Productive Meetings
Have an agenda
Stick to the time allotted
MEASURE performance!
If you are continually discussing the same things, you are making no progress and just wasting time.
Have a "Back to Work" time.

Same as a Triad (often called that in non-monogamy circles). Throuple is a more mainstream, monogamy reference to a Triad.
